This document presents a Math Mindset Survey designed to help students reflect on their feelings toward mathematics as they embark on a new unit. The survey is divided into two parts: the first section includes statements that students rate from 1 to 5 based on their self-assessment of their excitement, problem-solving attempts, and belief in growth through practice. The second part encourages students to reflect on their ratings and incorporate the concept of 'persistence' into their descriptions of how they feel about math. Additionally, there is a section called the 'Mathematician's Toolbox' which asks students to brainstorm tools and strategies they possess to support their growth as mathematicians. Lastly, students are prompted to connect their survey results to their attitudes about math and how these can influence their persistence throughout the unit.
