This is a great reference sheet to keep all year to surprise your students with their favorite candy or use the movie choices to compile a list of rewards day options! Keep GLOWING and GROWING Weekly Feedback Forms

Sending frequent feedback on a student's academic and behavior is essential to a student's growth. This form is the perfect way to let parents know areas a student is improving in as well as areas they could use additional support. On this form, both the parent and the student are required to sign, thereby also providing accountability for the student's behavior and allowing students to take ownership of their growth.

This product is in google forms. Send out link to parents and collect parent information regarding their child at the beginning of the year. Collects parents emails, phone numbers, best time to reach them at, questions regarding social media use in the class and a spot for parents to describe their child and express concerns. This is editable, so you can change questions to suit your class. It is best if you download this form to your personal google drive as many boards restrict access to those outside of the board.
