Cochlear Implant Surgery Workbook for Deaf HOH

By Sign with me TOD

In this product, students will learn about the process of receiving cochlear implants. They can add a picture of themselves to the first page of the booklet. Students will learn or review what steps are needed to receive cochlear implants - a hearing test, going for a scan to determine if cochlear implants would benefit a Deaf individual, going to the hospital to have cochlear implant surgery, a doctor called an otolaryngologist performs cochlear implant surgery, and what happens after surgery, when cochlear implants are turned on and mapped.

Students will review how they can care for their cochlear implants upon receiving them - charging them, putting them in their case when they are not being used, and where they should not go.

Students will be able to draw a picture of themselves wearing their cochlear implant(s). Students can then circle the color of their cochlear implants on the sheets provided. If the color of their cochlear implant is not included in the product, students can color the cochlear implants(s) pictures to show what they look like. At the end of this section, students will complete a fill in the blanks and true/false vocabulary review based on what was covered in the product.

Students will then learn about facts about hearing loss and Deafness - Examples: some people wear one hearing aid, one cochlear implant, two cochlear implants, etc., some people who are Deaf and have cochlear implants may use American Sign Language to communicate, the parts of the ear are reviewed - outer ear, ear canal, eardrum, cochlea, and auditory nerve, and how they work together to help someone hear. Students will label the parts of an ear diagram that is included in this product, identify the differences between hearing aids and cochlear implants, learn the different parts of the internal and external components of cochlear implants and where they are located using a diagram. Students will have the opportunity to again complete a fill in the blanks using a key to review what was learned about hearing aids and cochlear implants, and label a cochlear implant using a key. Students will review how the parts of the cochlear implant work together that allows them to hear. Sound fields and FM systems are also reviewed in this product.

At the end of the product, there are three slides that include the sign language and pictures to help students understand the process that takes place to when having cochlear implant surgery and once surgery has been completed.

Executive Functioning Skills Checklist for 3rd-5th: A Comprehensive Tool for Special Education Professionals

This checklist is crafted to help special education professionals and behavior specialists assess and support elementary students in 3rd-5th grades across 12 essential executive functioning areas. With age-appropriate subskills, interventions, and sample goals for IEP, FBA, and BIP plans, this checklist is your go-to tool for helping students develop the skills they need to succeed academically and socially. From managing materials to handling emotional regulation and staying focused, this checklist equips educators with a clear pathway for addressing executive function deficits.

What Problem Does This Solve?
In 3rd-5th grades, executive functioning skills become increasingly critical as students face more complex assignments, shifting social dynamics, and growing responsibilities. Students with deficits in these areas may struggle with organization, impulse control, persistence, or following multi-step instructions. This checklist provides a structured method for identifying and addressing these needs, offering targeted interventions to build skills that are essential for learning and social interaction.

Who This Helps
Designed for teachers, specialists, and support staff working with 3rd-5th graders, this checklist is an invaluable tool for educators looking to support students who may have challenges in one or more executive functioning areas. Whether you are developing IEP goals, conducting an FBA, or creating a BIP, this checklist provides insight into students’ current skills and highlights areas that may need more focused support.

Time-Saving Solution
With this checklist, educators receive structured interventions and measurable sample goals tailored for elementary students. By providing pre-developed goals and interventions, this tool saves valuable time, allowing educators to focus on what matters most: supporting student growth. The clear, organized structure of the checklist allows teachers to efficiently assess each student’s needs, implement interventions, and track progress over time.

What’s Included?

Add flair and organization plus to Take Home Folders, Homework Folders, School Notebooks, Binder Coves and Learning Centers. This Wolf theme set will engage and motivate those learners who need something a little different. Editable Wolf Theme Binder Covers & Spines, Take Home Folder Covers, Wolf Book Labels, Tracking Sheets, Goal Setting Charts, Wolves Posters, Signs and Writing Paper (and more!) offer variety and value and will hopefully save you time and energy too!

This editable Take Home Folder and File collection includes 108 editable templates with a Wolf Theme to help motivate and organize those learners who benefit from a personalized approach using a theme that interests and engages them.

I designed this Wolf Themed Folder and Binder set to motivate the few learners in my class each year who had very specific interests, were reluctant to take their folders home and worked best with a clear structure to help them set goals and chart their progress. It made a real difference when my learners were excited and proud to have folders and inserts with a theme that interested them. It was the easiest and quickest way I found to engage those who needed something more personalized.

These mix-and-match templates can be used for everything from seating and tray signs, individual learning boxes, class signs, notes to home, notebook covers and labels and anything else you need or can think of. I have developed quite a few themes over the years to cater for different interests and will be adding these to my store.

Mix-and-match wood patterns and coordinating ink-savers give you lots of options, choice and combinations for this year and beyond and can be used for younger learners, learners with special needs or older students who may need motivation and support but not cutesy themes.

Personalize to suit your students and class organization routines, print what you need, cut as required, use and enjoy. I hope they will save you time and energy too!

New Student- Special Education Form

By Specifically Sped

When a new student comes to class there are several things that take priority to everything else. I made this document after enrolling several new students this year. It has simplified my life tremendously by having a guide to go by when speaking with parents of new students. It also helps tremendously to fill out one of these on each student at the beginning of the year so that paras and teachers have a quick guide to go by per student.
