This form allows students to give their honest opinions about their group member's efforts at the end of their group project.

This form includes 5 simple questions with only 3 answer choices: YES, SOMETIMES, and NO.

There is also a section to give an overall star rating to each group member's effort (ranging from 1 to 5 stars), as well as an "Additional Comments" section.

This is also a good chance to have students evaluate themselves!
