Students will be excited to receive an award for their hard work, and parents, guardians, and gen ed teachers will know the goal has been met. Includes a place to write which goal has been mastered. Prints as a 5x7, so two print per page. Use colorful cardstock to make it pop!

Students can complete this behavior tracker to show how well they were able to:

I added room for the teacher to write in how many (if any) prompts are acceptable to follow directions and stay on task. I also left room for a percentage of work that should be completed. I know often my special education students struggle to complete assignments due to lack of focus, etc. So I might put that one student needs to complete 70% of his assignment and then gradually increase it. There is space for that to be written in in the chart.

Students can choose a big smiley face, a small smiling face, and a frowning face to show how well they were able to maintain the directives. At the bottom there is room for teacher signature and comments. At the top, the student has space to write what assignment he or she is assessing.

Two sheets print per page to save paper.

Level 2:

• Worksheets: Feature more advanced tasks including writing the word and its definition, choosing the correct picture from multiple options, selecting the appropriate item or tool, and using the word in a sentence.
• Assessments: Involve written responses using a word bank with a choice of 10 options, and matching tasks where students draw arrows to connect related items with a choice of 5 options.

An eco-map is often used an assessment tool in clinical mental health settings, but can be a beneficial SEL activity for social awareness. This exercise can be modified for small children all the way through adults.

The graph asks the participant to picture their social life as an ecosystem where they are at the center. In the blank space around them they can write in all of their formal and informal social ties and resources they have. (For younger children I suggest offering the option to draw the people in their life!) For further reflection, the participant can then add a + to signify if the person/resource is a positive part of their life, a - to signify if the person/resource is a negative part of their life, or write +- to signify a give and take relationship.

I use this worksheet along with social awareness lessons to encourage students to think about who they can ask for help from, who they have as supports, or reflect on relationships. It also allows you to gain insight into your students' life and resources outside of the classroom.
